The price isn’t the price. Here’s the machine behind your bill.

Every range on this site has a low end and a high end. The gap between them isn’t random — it’s how the system is built. Three things explain most of it.

1. The low end keeps the lights on

All over the country, imaging centers, labs, surgery centers, and independent practices run profitable businesses charging prices at the low end of our ranges — cash prices, posted up front. When a facility is structured around straightforward cash pricing, that price covers the equipment, the staff, and a margin. Nobody is doing you a favor at the low end. It’s a real price, offered every day, by businesses that plan to be here next year.

2. The insurance rate carries the overhead

When you buy care at the rates insurance companies agree to pay doctors and hospitals, you aren’t just buying the procedure — you’re buying the machinery between you and the procedure: the billing departments, the prior authorizations, the claims processing, the appeals, the administrators hired to argue with other administrators. That machinery has grown for fifty years, and it’s baked into the negotiated price.

3. The 20% rule — why bigger bills are better for them

Insurance companies are limited to keeping roughly 20 cents of every premium dollar for overhead and profit. Read that again from the insurer’s side of the table: the only way to grow the 20% is to grow the dollar. The more they agree to pay for a procedure, the bigger the pool their 20% comes out of. Higher negotiated prices aren’t a failure of the system — for the people negotiating them, they’re the business model.
